This is a Gorgeous circa World War I Italian Order of St. Gregory the Great Grand Cross Breast Star in Gilt & Enameled Silver by Tanfani & Bertarelli.
The Grand Cross Breast Star of the Order of St. Gregory the Great is a high-ranking award within the Vatican's Equestrian Order, specifically its Civil Division. It's a multi-piece construction, typically made of silver gilt with red and blue enamels and features a Maltese cross with ball finials. The breast star is often associated with Tanfani & Bertarelli, a well-known maker of Vatican Orders and decorations.
